This is critical because sustained business success in the evolving healthcare market will depend as much on organizational and systems alignment as on clinical practice. With the current level of operators and no clinical model, such alignment has been impossible to achieve in the current market place. Partnering with post-acute providers, we remake the acute care/post acute care relationship from the ground up into systematically aligned outcomes organizations performing at a level of excellence not achieved previously. This is critical for patient success while allowing all participating entities to partake in new financial opportunities in their local healthcare economies.
Sixteen years of research has proven that transitioning patients through acute and post acute entities - housing them, feeding them, giving them meds, exercises and walking them - without achieving the necessary changes in metrics for where they are returning only delays the readmission process. All parts of the continuum must identify and agree upon the necessary outputs to be achieved for a patient. Then each part of the continuum must facilitate achievement of the critical outputs relevant to their part of the continuum.
